City and County of San Francisco

For more than two decades, Homebridge has been the sole provider of IHSS contract mode services and workforce training to the City and County of San Francisco's Human Services Agency. For the past several years, Homebridge has also held contracts for case management with the Office on Aging and for workforce training and placement with the Office of Economic and Workforce Development. 


Health Plan of San Mateo

Homebridge partners with the Health Plan of San Mateo to provide in-home client care and case management for clients authorized by the County of San Mateo who are aged, blind, or disabled individuals unable to remain safely in their own homes without this assistance. 


Sutter Health

Sutter Health and Homebridge have partnered to develop a program of transitional, non-medical care to patients discharged from both the Emergency and in-patient departments of Sutter. Homebridge's home care providers accompanies these patients from the hospital to their home and provide ongoing follow up care including field based assessments and case management work to stabilize clients' in their homes and reduce readmisisons.

Metta Fund

Metta Fund is a philanthropic organization whose mission is to improve health in the City and County of San Francisco. Metta Fund provided funds to expand Homebridge Online's personal care giver registry program. Additionally, Metta Fund has provided valuable general operating support.

The Walter & Elise Haas Fund believes in collaborating with partners whose work provides access and opportunity, building a more equitable community. They invest in their Bay Area neighbors because doing so fosters a more just and vibrant society for current and future generations. 


Women's Foundation of California

Women's Foundation of California trains and invests in women to become policy advocates and philanthropic leaders who strengthen the economic well being of California's women and their families. As part of the Foundation's Bridge to Living Wage Initiative, a strategic effort to ensure economic security for low-income women and families in California, the Foundation provides funding to Homebridge to improve retention and success for home care providers through a full-time work life success coach counseling position and financial literacy trainings. 


Silicon Valley Community Foundation

Silicon Valley Community Foundation advances innovative philanthropic solutions to challenge problems. Homebridge is proud to have received funding from the Community Opportunity Fund of Silicon Valley Community Foundation to provide accessible, high-quality training, education, and resources for Home Care Providers in San Mateo.

We envision a future where high-quality, affordable health care and supports for daily living are delivered on each person's own terms, according to that individual's needs, values, and preferences.

Del E. Webb Foundation

Del E. Webb Foundation's mission is to provide support in the areas of medical services, research, and education to provide the most long term benefit for the most people through medicine and education. The Foundation funds Homebridge's growth and development of our Transitional Care Program that aims to reduce hospital admissions by providing supported transitions and ongoing support from hospitals to the home.  


Joseph & Vera Long Foundation

The Joseph & Vera Long Foundation is a private family foundation that provides support to not-for-profit organizations involved with healthcare, education, and conservation in the communities of Northern California and Hawai'i. The Foundation supports the growth and expansion of Homebridge Online's care giver registry, providing high-quality, competitively priced home care to consumers and new employment opportunities for care givers. 


Henry Mayo Newhall Foundation

The Henry Mayo Newhall Foundation is a private family foundation whose mission is to improve the quality of life within the geographical areas associated with the career and legacy of California pioneer, Henry Mayo Newhall. The Foundation is a supporter of Homebridge's transitional care program which includes the support of Homebridge Online. 


Northern California Community Loan Fund

The Northern California Community Loan Fund's mission is to promote economic justice and alleviate poverty by increasing the financial resilience and sustainability of community-based nonprofits and enterprises. The Northern California Community Loan Fund, in partnership with the Urban Solutions, has supported Homebridge through a Nonprofit Displacement Mitigation Program grant.
