City of San Francisco’s Department of Disability and Aging Services

For more than two decades, Homebridge has been the sole provider of IHSS contract-mode in-home services, and workforce training to the San Francisco Human Services Agency. In addition, Homebridge holds contracts for case management through the Office of Community Partnerships. These programs serving the community are funded by the City of San Francisco’s Department of Disability and Aging Services.

San Francisco Office of Economic and Workforce Development

Homebridge also holds contracts for workforce training and placement with the San Francisco Office of Economic and Workforce Development.

 

Metta Fund 

Metta Fund is a philanthropic organization whose mission is to improve health in the City and County of San Francisco. Metta Fund provided funds to expand Homebridge Online's personal care giver registry program. Additionally, Metta Fund has provided valuable general operating support.

 Walter & Elise Haas Fund

The Walter & Elise Haas Fund believes in collaborating with partners whose work provides access and opportunity, building a more equitable community. They invest in their Bay Area neighbors because doing so fosters a more just and vibrant society for current and future generations. 

Women's Foundation of California

The Women's Foundation of California trains and invests in women to become policy advocates and philanthropic leaders who strengthen the economic well being of California's women and their families. As part of the Foundation's Bridge to Living Wage Initiative, a strategic effort to ensure economic security for low-income women and families in California, the Foundation provides funding to Homebridge to improve retention and success for home care providers through a full-time work life success coach counseling position and financial literacy trainings.

The SCAN Foundation

The SCAN Foundation is an independent public charity devoted to transforming care for older adults in ways that preserve dignity and encourage independence. We envision a future where high-quality, affordable health care and supports for daily living are delivered on each person's own terms, according to that individual's needs, values, and preferences.

California Collaborative for Long Term Services and Supports

Homebridge is the fiscal sponsor of CCLTSS, the California Collaborative for Long Term Services and Supports. The Collaborative is a coalition of statewide organizations serving seniors and persons with disabilities. These organizations have agreed to a set of principles and meet regularly to exchange information and to work toward consensus on the design of an integrated system of long term services and supports in California. The Dignity Fund Coalition

The Dignity Fund Coalition seeks to ensure San Francisco seniors and adults with disabilities are able to live with dignity, independence, and choice in their homes.

SF Long Term Care Coordinating Council

San Francisco's Long Term Care Coordinating Council (LTCCC) advises the Mayor and City on policy, planning, and service delivery issues for older adults and people with disabilities to promote an integrated and accessible long-term care system. LTCCC has 40 membership slots that represent a variety of consumers, advocates, and service providers (non-profit and public).

Sutter Health

Sutter Health and Homebridge partnered to develop a program of transitional, non-medical care to patients discharged from both the Emergency and in-patient departments of Sutter. Homebridge's Caregivers accompanied these patients from the hospital to their home and provided ongoing follow up care including field based assessments and case management work to stabilize clients' in their homes and reduce readmisisons.

 Northern California Community Loan Fund

The Northern California Community Loan Fund's mission is to promote economic justice and alleviate poverty by increasing the financial resilience and sustainability of community-based nonprofits and enterprises. The Northern California Community Loan Fund, in partnership with the Urban Solutions, has supported Homebridge through a Nonprofit Displacement Mitigation Program grant.

 
 

The Seligman Group

Seligman Group develops, acquires, and manages commercial and residential properties throughout the western United States. Their Family Foundation, founded in 1987, is an annual funder of Homebridge.

First Republic Bank

Homebridge was proud to partner with First Republic Bank during Homebridge for the Holidays, our annual client gift campaign during the holiday season. In 2017, with First Republic Bank’s support, we were able to give nearly 400 individualized gifts that added richness to the lives of Homebridge clients. Found in 1985, First Republic and its subsidiaries offer private banking, private business banking, and private wealth management, including investment, trust, and brokerage services.
